As part of your application for a CDL, send this medical clearance form to the Wyoming DOT. MUST be filled out by a doctor or nurse.
OpenCDL 002 - Application for Commercial Driving Privileges
Application to the Nevada Department of Motor Vehicles for a new, renewed, or duplicate Class A, B, C, or M business driver's license OR commercial teaching permit.
CDL-004 - CDL Certification for Waiver of Skills Tests
If you just got out of the service and drove a commercial car, you can use this form to ask to skip the skills test when you apply for a Nevada CDL.
